Updating the Index after a lapse of twelve years was a massive task that would have been impossible without the unstinted support of numerous colleagues. We owe an especial debt of gratitude to Michael for responding to our numerous queries promptly and expertly. Martin Head reviewed the Introduction and made many helpful suggestions, and we also thank Jennifer Galloway for reviewing the new material. We are indebted to Bill MacMillan and Nelly Koziel for their tireless support, as always, with compilation and production.
